Episode 2 - Heathrow Crash Landing

  • On 17 January 2008, during short final both British Airways Flight 38's engines rolled back to idle. The Captain raises the flaps which give them a few more feet of flying time. The aircraft passes over the A30 road and crashes just short of the runway. There were no fatalities, but 47 people sustained injuries. The fuel flow to both engines was restricted because of ice crystals causing a blockage in the fuel-oil heat exchangers (FOHE). Another similar incident happened On November 26, 2008 when Delta Air Lines Flight 18's engines also rolled back to idle at 39,000 feet. That plane landed safely in Atlanta.

Episode 3 - Pilot Betrayed

  • A Douglas MD-81 has been de-iced twice, but in less than two minutes after take-off, both engines fail within 2 seconds. As the pilots try restarting them the left engine erupts in flames, and then 3,200 feet above the ground, the plane quickly and silently falls from the sky. The plane breaks into three pieces and crashes into a field, but remarkably everyone on board survives. The cause was clear ice being sucked into both engines when a system known as Automatic Thrust Restoration increased engine output after the engines had been set to idle in an attempt to save them. The ATR system's existence wasn't something the pilots were informed about.

The flight just lasted about 5 minutes when the plane ditched into the Hudson River, amazingly leaving all onboard alive. The ditching is described as "the most successful ditching of all time".
